Chronic disease management

Managing chronic diseases can be very challenging, however, people who take an active role in participating in a chronic condition management program usually enjoy a more active and fulfilling lifestyle. Our specialty programs focus on involving patients in the health care process so they can achieve more independence and a better quality of life. 

The goal of each of our programs is to reduce hospitalizations and keep our patients comfortable in their own homes. Our plans of care include the most current medical evidence, RPM (remote patient monitoring) technology and clinical resources to ensure maximum success with each patient.


RehabPlus™ Home Therapy

A combination of skilled nursing care to treat the surgical wound and therapeutic rehabilitation to address the internal joint or implant all in the comfort of the patient’s own home provides the optimal environment for recovery after an orthopedic surgery or injury. Our goal is to assist the patient in reaching the highest possible level of independence in the comfort of their home.  We facilitate necessary Durable Medical Equipment to be delivered to your home, process parking permits for temporary handicap parking access and, of course, provide physical and occupational therapies for a speedy recovery and restored independence.

 

Fall Prevention

Falls are the leading cause of nonfatal, unintentional injuries treated in a hospital ER department–and the leading reason for admission to a skilled nursing facility. Our Fall Prevention program is a comprehensive program that evaluates each patient to develop a systematic approach to treatment. The goal is restoration and management of the balance disorder to facilitate safe, independent living within the home.

Wound Care

Wounds can be very complicated and usually vary greatly from patient to patient which is why our CWS-certified wound care nurses are so important in designing wound care regime specific to your condition. We include in our Medline-based formulary only the best and most current advanced wound care products available to help reduce recovery time and manage discomfort.


Diabetes Management and Education

There are two types of Diabetes, Type I and Type II, and they require different approaches to managing the conditions of each. There have also been recent advances in treatment options for Diabetes and the education-based focus we provide our patients utilizes this new knowledge including diet and nutrition along with medication. Kamcare Home Health Services clinicians focuses on each patient’s specific needs, improving quality of life and enabling patients to achieve their best health status.


COPD/Heart Failure

Living with Chronic Obstructive Pulmonary Disease (COPD) and/or Heart Failure (HF) is stressful for patients and families alike. Our nurses provide a combination of skilled care, therapy and disease process and nebulizer education to improve patients’ ability to manage COPD/HF and enjoy a higher quality of life. Patients and caregivers are encouraged and motivated to actively participate in creating the management plan and educated in the skills needed to satisfy each component of the plan.


Chronic Kidney Disease

Patients and families coping with Chronic Kidney Disease have the support of Kamcare Home Health Services clinicians who provide ongoing assessment, timely treatment and coordination with physicians, renal clinics and other health care providers. Our skilled clinicians provide information and education to support the patient and family, improve the patient’s quality of life and decrease the overall stress level that this condition creates.
