Home Installation

One of our RN Case Managers will bring the monitoring equimpment to the patient’s home. Set up is so simple it literally only involves plugging in the Hub (data communication device) into a standard outlet. Based on the patient’s care plan, certain devices (i.e. blood pressure cuff, scale, pulse oximeter) will be assigned to the patient and the RN will pair the devices to the Hub.

 

Use Your Devices

Patients just need to begin using the devices. That’s it! Your devices will do all the rest and your personal data will automatically be transmitted by the Hub to our cloud-based portal for us to keep a close watch over.

Our RPM Clinical Manager will monitor the patient data for anything that looks like it might need extra attention. Most of the time all that is needed is a polite reminder to use the devices but should an extra visit by one of our nurses be required, we’ll send one out right away!
