Chronic Care Programs

Living with a chronic disease can be challenging, but patients who take an active role in their own care can make more informed choices and are less likely to be re-hospitalized down the road. Our Chronic Condition Management programs focus on involving patients in the health care process so they can achieve more independence and a better quality of life.

The goal of each of our programs is to decrease hospitalizations for the long term. We use the most current medical evidence to develop our clinical and patient education, and plans for patient care.

Wound Care

Wounds can be very complicated and usually vary greatly from patient to patient which is why our certified wound care nurses are so important in designing wound care regime specific to your condition. We use only the best and most current wound care products available to help reduce recovery time and manage discomfort.


There are two types of Diabetes, Type I and Type II, and they require different approaches to managing the conditions of each. There have also been recent advances in treatment options for Diabetes and the education-based focus we provide our patients utilizes this new knowledge including diet and nutrition along with medication. Kamcare Home Health Services clinicians focuses on each patient’s specific needs, improving quality of life and enabling patients to achieve their best health status.

Rehab Plus

A combination of skilled nursing care to treat the surgical wound and therapeutic rehabilitation to address the internal joint or implant all in the comfort of the patient’s own home provides the optimal environment for recovery after an orthopedic surgery or injury. Heart in the Home

We manage all of our heart condition patients using our Heart in the Home program. Kamcare Home Health Services clinicians provide detailed assessment and education to help patients successfully manage medication, symptoms, mobility and all other components of their care. We often coordinate other services, including physical therapy, special equipment and nutritional education. Our goal is to ensure our patients have the information and educational tools to remain safe in their home after home health care has ended.


Living with Chronic Obstructive Pulmonary Disease (COPD) is stressful for patients and families alike. Our nurses provide care, therapy and education to improve patients’ ability to manage COPD and enjoy a higher quality of life. Patients and caregivers are encouraged and motivated to actively participate in creating the management plan and educated in the skills needed to satisfy each component of the plan.

Chronic Kidney Disease

Patients and families coping with Chronic Kidney Disease have the support of Kamcare Home Health Services clinicians who provide ongoing assessment, timely treatment and coordination with physicians, renal clinics and other health care providers. Our skilled clinicians provide information and education to support the patient and family, improve the patient’s quality of life and decrease the overall stress level that this condition creates. With reliable information about the disease and its management, patients with Chronic Kidney Disease can control their condition and minimize its impact.

Pain Management at Home

Chronic pain can be debilitating and agonizing if left untreated and it significantly impacts quality of life for patients and their families. Our pain management program is designed to actively involving all parties involved including not just the patients themselves but also caregivers, nurses and any other therapeutic discipline needed. Together, we develop a holistic plan of care based on each patient’s individual condition. Using the best clinical evidence currently available, Kamcare Home Health Services clinicians work with the patient and family, teaching the skills needed to help them continue to control pain after the home health care has ended.

Balance is the Key

Falls are the leading cause of nonfatal, unintentional injuries treated in a hospital ER department–and the leading reason for admission to a skilled nursing facility. In the U.S. alone, one of three adults over the age of 65 falls each year, and more than half fall more than once. This annual rate increases to one of two over the age of 80. Kamcare Home Health Services Balanced is the Key Program is a comprehensive program that evaluates each patient to develop a systematic approach to treatment. The goal is restoration and management of the balance disorder to facilitate safe, independent living within the home.
